ABSTRACT
Background: Intrathecal clonidine prolongs spinal anesthesia and postoperative analgesia when added to hyperbaric bupivacaine. However, its optimal dose for caesarean section remains uncertain.
Objective: To compare the efficacy and safety of intrathecal clonidine 15 µg and 30 µg as adjuvants to hyperbaric bupivacaine in caesarean section.
Materials and Methods: This prospective, randomized, double-blind comparative study involved 30 parturients divided equally into Group A (15 µg clonidine) and Group B (30 µg clonidine), each receiving 10 mg of 0.5% hyperbaric bupivacaine. Sensory and motor blockade, hemodynamic parameters, postoperative analgesia, and adverse effects were evaluated.
Results: In the dataset, two-segment sensory regression was significantly prolonged in Group B compared with Group A (145.8 ± 21.4 vs. 112.4 ± 18.6 minutes; p<0.001). Motor blockade duration was longer (169.7 ± 22.6 vs. 138.5 ± 19.4 minutes; p<0.001). Time to first rescue analgesia was 278.4 ± 36.2 versus 208.6 ± 29.4 minutes (p<0.001). Hypotension occurred in 33.3% versus 13.3% of patients (p=0.390).
Conclusion: Intrathecal clonidine 30 µg may provide prolonged postoperative analgesia and blockade compared with 15 µg, although maternal hemodynamic monitoring remains essential.
